Separating Fact from Fiction
Testosterone replacement therapy is surrounded by misconceptions that can prevent men from seeking the treatment they need. Here we address the most common myths and provide the facts, backed by medical evidence and clinical experience.
Myth: TRT Causes "Roid Rage"
The Truth: This is one of the most persistent myths about testosterone therapy. The concept of "roid rage" is associated with the abuse of supraphysiological doses of anabolic steroids, not with medically supervised TRT. Therapeutic TRT aims to restore testosterone to normal, healthy levels. In fact, many men report improved mood stability and reduced irritability once their levels are optimised. Low testosterone is far more commonly associated with mood swings and irritability than normal levels are.
Myth: TRT is the Same as Taking Steroids
The Truth: While testosterone is technically a steroid hormone, medically prescribed TRT is fundamentally different from anabolic steroid abuse. TRT uses carefully controlled doses to bring testosterone levels back into the normal physiological range. Steroid abuse involves taking many times the normal dose, often combining multiple compounds, to achieve levels far beyond what the body would naturally produce. TRT is a legitimate medical treatment prescribed by doctors for a diagnosed condition.
Myth: TRT is Only for Old Men
The Truth: Low testosterone can affect men of any age. While levels naturally decline with age, younger men can also experience hypogonadism due to genetic factors, medical conditions, lifestyle issues or previous use of anabolic steroids. We see patients across a wide age range at TRT South, from men in their late 20s to those in their 70s. If your blood work shows low testosterone and you are experiencing symptoms, age is not a barrier to treatment.
Myth: TRT Causes Heart Problems
The Truth: Early studies raised concerns about a potential link between TRT and cardiovascular risk, but more recent and robust research has largely dispelled these fears. A major 2023 study (the TRAVERSE trial) involving over 5,000 men found no increased risk of major cardiovascular events in men receiving TRT compared to placebo. In fact, low testosterone itself is associated with increased cardiovascular risk factors including obesity, insulin resistance and metabolic syndrome. Properly managed TRT, with regular monitoring, is considered safe for most men.
Myth: TRT Will Make You Infertile Permanently
The Truth: TRT can reduce sperm production, and in some cases may cause temporary infertility. However, this is usually reversible once treatment is stopped, and can often be mitigated by including hCG in the treatment protocol. At TRT South, we always discuss fertility goals with patients before starting treatment and can design protocols that help preserve sperm production. Permanent infertility from TRT alone is extremely rare.
Myth: You Can Never Come Off TRT
The Truth: While many men choose to remain on TRT long-term because of the benefits, it is possible to discontinue treatment. Your natural testosterone production may take some time to recover, and our doctors can guide you through a structured tapering protocol if you decide to stop. The decision to continue or stop TRT is always yours to make, in consultation with your medical team.
Myth: TRT Causes Prostate Cancer
The Truth: This is an outdated belief that has been thoroughly debunked by modern research. Multiple large-scale studies have found no evidence that TRT increases the risk of prostate cancer. Current guidelines from major urological and endocrine societies state that TRT is not contraindicated in men without a current diagnosis of prostate cancer. At TRT South, we include PSA monitoring in our regular blood panels as a standard precaution.
Myth: Natural Supplements Can Replace TRT
The Truth: While a healthy lifestyle can support testosterone production, no over-the-counter supplement has been proven to significantly raise testosterone levels in men with diagnosed hypogonadism. Many "testosterone booster" products contain ingredients with little or no clinical evidence behind them. If your blood work shows genuinely low testosterone, medically prescribed TRT is the only reliable way to restore your levels to the optimal range.
