What is hCG?
Human Chorionic Gonadotropin (hCG) is a naturally occurring hormone that mimics the action of Luteinising Hormone (LH) in the body. LH is the hormone responsible for signalling the testes to produce testosterone and maintain sperm production. When used as part of a TRT protocol, hCG helps to maintain the body's natural processes that would otherwise be suppressed by exogenous testosterone.
Why is hCG Used Alongside TRT?
When a man starts testosterone replacement therapy, his body receives testosterone from an external source. The brain detects these elevated hormone levels and reduces its own signals (LH and FSH) to the testes. Over time, this can lead to testicular atrophy (shrinkage) and a significant reduction in sperm production, potentially causing infertility.
hCG addresses this issue by directly stimulating the testes to continue functioning, even while the brain's natural signals are suppressed. This makes it an essential component of TRT for many men, particularly those who wish to preserve their fertility.
Preserving Fertility
For men who are planning to have children in the future, or who simply want to keep their options open, hCG is a crucial addition to their TRT protocol. Without it, sperm production can drop to very low levels or cease entirely. By including hCG in the treatment plan, we can help maintain sperm production throughout the course of TRT.
It is important to note that while hCG significantly improves the chances of maintaining fertility, it is not a guarantee. Men who are actively trying to conceive should discuss their specific situation with our doctors so that we can tailor the protocol accordingly.
Maintaining Testicular Function
Beyond fertility, hCG helps to prevent testicular atrophy - a common and often distressing side effect of TRT. Many men find that maintaining testicular size is important to them both physically and psychologically. hCG keeps the testes active and functioning, which also supports the production of other important hormones and precursors produced within the testes.
How TRT South Manages hCG and TRT
At TRT South, we take a comprehensive approach to hormone management. Our doctors will discuss the role of hCG during your initial consultation and determine whether it should be included in your treatment plan based on your individual circumstances, goals and blood work.
Key aspects of our approach include:
- Thorough assessment of your fertility goals and future plans
- Careful dosing of hCG tailored to your individual response
- Regular blood work monitoring to ensure optimal levels of testosterone, LH, FSH and oestradiol
- Ongoing protocol adjustments based on your results and how you feel
- Clear communication about what to expect and how to administer hCG
Common Questions About hCG and TRT
How is hCG administered? hCG is typically given as a subcutaneous injection, usually two to three times per week. Our team will teach you how to self-administer these injections safely and painlessly.
Are there side effects? hCG is generally well tolerated. Some men may experience mild water retention or slight mood changes. Our monitoring protocols help us identify and manage any side effects quickly.
Does everyone on TRT need hCG? Not necessarily. Men who have completed their families and are not concerned about testicular atrophy may choose not to include hCG in their protocol. This is always a personal decision made in consultation with your doctor.
